Youth Transition Planning Services

RAMP partners with local school districts to enhance the outcomes of high school students with disabilities. Through this partnership RAMP provides resource for school districts, students and families, completes individualized assessments that looks at the student’s post high school desires and then provides the school district and others with ideas on how to meet these desires. RAMP staff also draft goals in the areas of post-secondary education, employment, and independent living, and assists the student and their families with making those important community connections.

School Districts often do not have the time or resources to provide students with the transition services they need to meet their post-secondary goals.
